Community Networks of Care

Many seniors in Pennsylvania would prefer to be in their homes as long as possible, but in order to do so, they need better access to, and active participation in, the services they receive.

United Way is committed to keeping seniors safe and healthy in their homes. That’s why UWSEPA, in partnership with the Pennsylvania Department of Aging, has created Community Networks of Care (CNC). CNC is a program designed to improve the delivery of home and community-based services for older adults allowing seniors to remain living independently in their homes through a model of “community living”.

UWSEPA and the Department of Aging are working to develop a Community Network of Care in our region and will be developing two more communities in the next three years.
